The NCCN provides comprehensive guidelines that cover various aspects of breast cancer, including prevention, screening, diagnosis, and treatment. Guidelines for breast cancer prevention may include recommendations related to:
The Nccn Offers Recommendations For A Range Of Cancer Forms, From Early Identification And Prevention To Treatment And Aftercare. Expert Committees Made Up Of Medical Experts Created These Evidence-based Recommendations. Oncologists, Medical Professionals, And Organizations Utilize Them Extensively To Direct Clinical Judgment And Guarantee Excellent Cancer Treatment.
Seek Advice From Medical Experts Who Have Access To The Most Recent Recommendations Or Go To The Official Nccn Website For The Most Up-to-date And Accurate Information On Nccn Guidelines For Breast Cancer Or Any Other Kind Of Cancer.
The History And Physical Examination; Bilateral Diagnostic Mammography; Pathology Review; Tumor Estrogen Receptor (Er) Status Determination; And Mri, As Indicated, Are Included In The Recommended Workup And Staging Of Dcis. The College Of American Pathologists (Cap) Protocol For Both Invasive And Noninvasive Breast Cancers Is Endorsed By The Nccnn Breast Cancer Panel For Pathology Reporting.2.
To Ascertain If Adjuvant Endocrine Therapy Or Risk Reduction Is Beneficial, The Nccn Panel Advises Testing For Erythrocyte Status. This Is Compliant With The Asco/cap Guidelines3, Which Suggest That Progesterone Receptor (Pr) Testing Be Taken Into Consideration And That Er Testing Of Recently Diagnosed Dcis Be Considered Optional To Assess Potential Benefit Of Endocrine Therapies For Breast Cancer Risk Reduction.
The Tumor Her2 Status Has Not Been Determined To Be Important In Dcis, Despite Being Of Prognostic Significance In Invasive Cancer. Trastuzumab Used In Conjunction With Radiation In Her2 Amplified Dcis Does Not Seem To Provide A Statistically Significant Benefit, According To Studies Conducted To Date. Her2 Status As A Prognostic Indicator In Dcis Has Also Been Shown To Be Unclear Or Weak.8 The Ncccn Panel Has Determined That Dcis’s Her2 Status Is Not Advised Since It Does Not Change The Management Strategy.
The Nccnn Guidelines For Genetic/familial High-risk Assessment: Breast, Ovarian, And Pancreatic Cancer (Available At Nccn.org) Consider The Patient To Be At High Risk For Inherited Breast Cancer. In This Case, Genetic Counseling Is Advised.
It’s Still Unclear How Mri Fits Into The Dcis Management Structure. Prospectively, It Has Been Shown That Mri Is Up To 98% Sensitive To High-grade Dcis.9. In A Prospective Observational Study Of 193 Patients With Pure Dcis Who Had Preoperative Mammography And Mris, 93 Patients (56%) Had A Mammography Diagnosis And 153 Patients (92%) Received An Mri Diagnosis (P<.0001).
Mri Alone Made The Diagnosis For 43 (48%) Of The 89 Patients With High-grade Dcis Who Were Not Diagnosed By Mammography.9. Other Research, However, Seems To Indicate That Mri May Overestimate The Severity Of Illness.
10 Consequently, Mri Findings Alone Should Not Be The Only Basis For Surgical Decisions Regarding The Performance Of A Mastectomy For Dcis. Mri-guided Biopsies Of The More Extensive Enhancement Should Be Used To Confirm The Findings Histologically If The Results Indicate A More Extensive Disease Than What Is Seen On Mammography And A Much Larger Resection Is Needed For Complete Excision. Research Done To See If Using Mri Lowers Local Recurrence And Re-excision Rates In Patients With Dcis Yields Contradictory Results.
While A Number Of Studies Indicate That Patients With Pure Dcis Undergoing Breast-conserving Surgery (Bcs) After Mri Did Not See A Decrease In Re-excision Rates When Compared To Those Who Did Not Have Preoperative Mri,11,12 Some Have Shown That Using Preoperative Mri For Dcis Reduces The Re-excision Rate.13, 14 A Study Found That 6.2% More Cancers Were Detected When Preoperative Mi Was Used.14 Thus, The Use Of Preoperative Mri Is Still Debatable.
The Nccn Panel Advises Against Using Breast Magnetic Resonance Imaging (Mri) For Dcis Unless There Is A Clear Indication During The First Workup That Further Information Is Needed. Mri Has Not Been Shown To Reduce The Likelihood Of Negative Margins Or To Decrease The Conversion To Mastectomy For Dcis.
